Velpatasvir is a direct-acting antiviral that inhibits NS5A, a hepatitis C virus protein needed for viral replication and assembly. It is active against all six major hepatitis C genotypes.
Velpatasvir is only available in combination. With sofosbuvir (originator brand Epclusa) it is approved for chronic hepatitis C virus infection of any genotype in adults and children, including patients with compensated cirrhosis and, in combination with ribavirin, decompensated cirrhosis. With sofosbuvir and voxilaprevir (Vosevi) it is approved for some patients previously treated with direct-acting antivirals. Sofosbuvir with velpatasvir is on the WHO Model List of Essential Medicines and is a pan-genotypic regimen recommended by the WHO.
