Bosutinib is a second-generation tyrosine kinase inhibitor that blocks the BCR-ABL fusion protein of the Philadelphia chromosome and SRC-family kinases. It has relatively little effect on KIT and PDGFR, which gives it a different side-effect profile from imatinib.
It is approved for newly diagnosed chronic-phase Philadelphia chromosome-positive chronic myeloid leukaemia (CML), and for chronic, accelerated or blast-phase CML in patients previously treated with one or more tyrosine kinase inhibitors, including where imatinib, nilotinib and dasatinib are not considered appropriate. In the United States approval also covers children with chronic-phase CML. The originator brand is Bosulif.
Diarrhoea and liver enzyme changes are common early in treatment and are managed by the haematologist.
