Bendamustine is a bifunctional alkylating agent that combines a nitrogen mustard group with a purine-like benzimidazole ring. It cross-links DNA and kills both dividing and resting cancer cells.
It is approved for chronic lymphocytic leukaemia, in Europe as first-line treatment for patients for whom fludarabine combination chemotherapy is not appropriate, and for indolent B-cell non-Hodgkin lymphoma that has progressed during or within six months of rituximab treatment. In some European countries it is also approved, with prednisone, for multiple myeloma. In clinical practice it is widely combined with rituximab. Originator brands include Treanda, Levact and Ribomustin.
Bendamustine is given by intravenous infusion in hospital and handled under cytotoxic safe-handling procedures.
