Chlorambucil is an oral alkylating agent of the nitrogen mustard class. It cross-links DNA in dividing and resting cells and acts relatively slowly, which suits it to chronic blood cancers.
It is approved for chronic lymphocytic leukaemia and for malignant lymphomas, including indolent non-Hodgkin lymphoma and Hodgkin lymphoma; in some countries also for Waldenström's macroglobulinaemia. In older or less fit patients with chronic lymphocytic leukaemia it has been used in combination with anti-CD20 antibodies. Chlorambucil is on the WHO Model List of Essential Medicines, and the originator brand is Leukeran.
Chlorambucil tablets are kept refrigerated at 2°C to 8°C, so shipments need temperature-controlled packaging; this is unusual for an oral cancer medicine and worth planning for.
