Colistimethate sodium is an inactive prodrug of colistin (polymyxin E), a polymyxin antibiotic. Colistin binds the outer membrane of Gram-negative bacteria and disrupts it, killing the bacteria.
Intravenous colistimethate is approved for serious infections caused by aerobic Gram-negative bacteria, such as Pseudomonas aeruginosa, Acinetobacter baumannii and Klebsiella pneumoniae, when other antibiotics cannot be used; in practice it is a last-line treatment for multidrug-resistant infections. Inhaled colistimethate is approved for chronic Pseudomonas aeruginosa lung infection in patients with cystic fibrosis. Originator brands include Colomycin, Promixin and Coly-Mycin M. It is on the WHO Model List of Essential Medicines, in the Reserve group of antibiotics.
Colistimethate can damage the kidneys and nerves, so kidney function is monitored. Doses are expressed in international units; our catalog lists the 2 million IU vial.
