Doxorubicin is an anthracycline antibiotic. It intercalates into DNA and inhibits topoisomerase II, causing DNA breaks that kill dividing cells.
Conventional doxorubicin is approved for a wide range of cancers, including breast cancer, acute lymphoblastic and acute myeloid leukaemia, Hodgkin and non-Hodgkin lymphoma, soft tissue and bone sarcomas, Wilms' tumour, neuroblastoma, and ovarian, bladder, thyroid, gastric and small cell lung cancers. It is on the WHO Model List of Essential Medicines; the originator brand is Adriamycin. Pegylated liposomal doxorubicin (Caelyx, Doxil) is a different product approved for ovarian cancer, AIDS-related Kaposi sarcoma and multiple myeloma, and the two are not interchangeable.
Heart function is monitored because doxorubicin can damage the heart muscle, with risk rising with the total amount received over a lifetime.
