Exemestane is a steroidal aromatase inactivator. It binds aromatase irreversibly, so the enzyme can no longer convert androgens into oestrogens; this differs from anastrozole and letrozole, which block aromatase reversibly.
It is approved for postmenopausal women with oestrogen-receptor-positive early breast cancer, as adjuvant treatment after two to three years of tamoxifen, to complete five years of hormone therapy; and for advanced breast cancer that has progressed after anti-oestrogen therapy. In clinical practice it is also combined with everolimus in advanced breast cancer, an approved use of everolimus. The originator brand is Aromasin.
