Hydroxyurea, also called hydroxycarbamide, inhibits ribonucleotide reductase, the enzyme that makes the building blocks of DNA. This slows the production of blood cells and, in sickle cell disease, raises the level of fetal haemoglobin, which reduces the sickling of red cells.
It is approved for resistant chronic myeloid leukaemia and, in the United States, with radiotherapy for some squamous cell cancers of the head and neck. Separately labelled products are approved to reduce painful crises and the need for transfusion in sickle cell anaemia, in adults and children. In several European countries hydroxycarbamide is also approved for essential thrombocythaemia and polycythaemia vera. It is on the WHO Model List of Essential Medicines.
