Irinotecan is a semi-synthetic derivative of camptothecin. It and its active metabolite SN-38 inhibit topoisomerase I, the enzyme that relieves tension in DNA during replication, causing lethal DNA breaks.
Conventional irinotecan is approved for metastatic colorectal cancer, as first-line treatment in combination with fluorouracil and leucovorin, and after failure of fluorouracil-based treatment. In Europe it is also approved in combination with cetuximab or bevacizumab. The originator brand is Campto (Camptosar in the US). A liposomal formulation (Onivyde) is a different product approved for pancreatic cancer, and the two are not interchangeable.
Patients with reduced activity of the UGT1A1 enzyme clear SN-38 more slowly and may need a dose adjustment, and diarrhoea is a well-known side effect that oncologists manage.
