Pyrazinamide is an antituberculosis medicine, a derivative of nicotinamide. It is converted inside Mycobacterium tuberculosis to pyrazinoic acid and is particularly active against slowly multiplying bacteria in the acidic environment of inflamed tissue.
It is approved for the initial treatment of active tuberculosis, always in combination with other antituberculosis medicines. In standard WHO regimens pyrazinamide is given with isoniazid, rifampicin and ethambutol during the first two months of treatment, which allows the total treatment course to be shortened. It is also part of some regimens for drug-resistant tuberculosis. Pyrazinamide is on the WHO Model List of Essential Medicines.
Liver function and uric acid are monitored during treatment, as pyrazinamide can affect both.
