Vincristine is a vinca alkaloid derived from the Madagascar periwinkle. It binds tubulin and prevents the formation of the mitotic spindle, so cancer cells cannot divide.
It is approved, in combination regimens, for acute leukaemia, especially acute lymphoblastic leukaemia; for Hodgkin and non-Hodgkin lymphomas, for example in the CHOP regimen; and for rhabdomyosarcoma, neuroblastoma and Wilms' tumour. It is used widely in children's cancer treatment and is on the WHO Model List of Essential Medicines.
Vincristine is for intravenous use only and is fatal if given by other routes, particularly into the spinal fluid, so regulators require special labelling and hospitals follow strict handling procedures. Nerve damage is the main limiting side effect. Vincristine vials are stored refrigerated at 2°C to 8°C and protected from light, so shipments need temperature-controlled packaging.
