Dabigatran etexilate is a direct oral anticoagulant. It is a prodrug converted in the body to dabigatran, which directly inhibits thrombin, the enzyme that turns fibrinogen into fibrin in a blood clot.
It is approved to reduce the risk of stroke and systemic embolism in non-valvular atrial fibrillation; to treat deep vein thrombosis and pulmonary embolism and reduce the risk of recurrence; and to prevent venous thromboembolism after hip or knee replacement surgery. It is also approved for venous thromboembolism in children. A specific reversal agent, idarucizumab, is available. Dabigatran is not used in patients with mechanical heart valves. The originator brand is Pradaxa.
Dabigatran comes as capsules that are swallowed whole, not opened, and kept in their original pack to protect them from moisture.
