Diltiazem is a benzothiazepine calcium channel blocker. Unlike dihydropyridines such as amlodipine, it acts on the heart as well as the blood vessels, slowing conduction through the AV node and reducing heart rate and the force of contraction.
Oral diltiazem is approved for chronic stable angina and angina due to coronary artery spasm, and, as extended-release formulations, for hypertension. Intravenous diltiazem is approved for temporary control of a rapid ventricular rate in atrial fibrillation or atrial flutter and for converting certain supraventricular tachycardias. Oral diltiazem is also used to control heart rate in atrial fibrillation. Originator brands include Cardizem and Tildiem.
