Riociguat is a soluble guanylate cyclase (sGC) stimulator. It sensitises sGC to the body's own nitric oxide and also stimulates it directly, increasing cyclic GMP, which relaxes the blood vessels of the lungs.
It is approved for chronic thromboembolic pulmonary hypertension (CTEPH) that is inoperable or that persists or recurs after surgical treatment, and for pulmonary arterial hypertension, alone or with an endothelin receptor antagonist. It is the only medicine approved for inoperable CTEPH in many countries. The originator brand is Adempas.
Riociguat is not taken with nitrates or with phosphodiesterase-5 inhibitors such as sildenafil, because of the risk of very low blood pressure, and it can harm an unborn baby, so pregnancy prevention is required.
