Ritonavir is an HIV protease inhibitor used mainly as a pharmacokinetic booster used for HIV-1 infection, as a booster of other protease inhibitors. Ritonavir is an HIV protease inhibitor. At the low doses now used, its main role is as a pharmacokinetic booster: it strongly inhibits the liver enzyme CYP3A4, which raises and prolongs the blood levels of other protease inhibitors given with it.
It is approved for the treatment of HIV-1 infection in combination with other antiretrovirals, and is co-formulated or co-packed with protease inhibitors such as lopinavir (originator brand Kaletra), atazanavir and darunavir. The originator brand of ritonavir alone is Norvir. Ritonavir is also the booster in nirmatrelvir with ritonavir (Paxlovid), an oral COVID-19 treatment. Because it inhibits CYP3A4, ritonavir interacts with many other medicines, which prescribers check before starting it.
