Teriflunomide is an oral immunomodulator. It inhibits dihydroorotate dehydrogenase, a mitochondrial enzyme that rapidly dividing lymphocytes need to make pyrimidines, and so reduces the number of activated T and B cells that drive multiple sclerosis. It is the active metabolite of leflunomide.
It is approved for relapsing forms of multiple sclerosis in adults, including, in the United States, clinically isolated syndrome and active secondary progressive disease, and in Europe and other regions for children from ten years of age with relapsing-remitting multiple sclerosis. The originator brand is Aubagio.
Liver function and blood counts are monitored during treatment. Teriflunomide stays in the body for a long time and can harm an unborn baby, so pregnancy must be avoided, and an accelerated elimination procedure exists for patients who need to clear it quickly; these are decisions for the neurologist.
